1. Fill in the Registration Form

2. Fill in the Dealer Code Application Form
   (COMPULSORY if you want to earn money from your introductions)

3. Submit BOTH forms to:
   

   i.	U Mobile Centres ( For CRP 1 )
   ii.	Selected Mobile City Shops
